internal medicine - income/lifestyle as a hospitalist or subspecialists

if you are talking large volume community hospitals like BCH and Trillium then a GIM who does mainly ER work will likely make closer to 400-500 without ANY overheard. None. So even with taxes you still make 250-300. The donshfit work mainly so more nights but some MRP and clinic and consult ward work as well. Usually have about 7 days off a month. 

 

Endo and Rheum are similar but pay overhead.  No night calls thought and day clinic work, less stressful. 

 

Cardio and GI make much more in large centres. Most are 800-1.7million range depending on what they do.  They do have overhead but still take home much more (500-1mill).

 

ICU is another good biller with lots of time off. Likely 400-700 a year with no overhead. Work invokes one week a month ICU and some base specialty work like GIM but usually 7-14 days off a month 

 

 

Not staff, but these numbers really seem like outliers from my experience talking to people in these fields. If they indeed true, then BCH and Trillium are excellent places for work (in terms of income) for IM specialists.

Depends where you work and how much you work (days per week and volume).

I don't think we can all agree on one set of numbers because some people set their consult appointments longer, some shorter.

Some work in ERs, some work more in clinics. Some do more procedures or take more call shifts than others.

Gross billings, generally for a 60-70 hr per week internist you're looking at 400k+, and subspecialist with procedures it's 700k+.

 

Lifestyle is another story. Would you rather be the 800k biller working 80 hr weeks, or the guy billing 400k/ 40 hr weeks. Some people like family med for the reason you can get short days with high income ratio, job flexibility, etc

100 percent agree. Right on point. 

 

One of the best parts about medicine is that you can work as much or as little as you want. 

 

If you want to kill yourself and work 100 hour weeks to find private school and fancy vacations then you can. If you would rather make less but attend your sons T-ball games you can as well. 

 

There are some legends in ER that see upwards of 150 patients a day and work 16 hour days 25-30 times a month. There are others working and seeing 40 patients a shift working 10 times a month.

 

If we want to work like dogs we can. No rules on how much you can work. On rules on overtime or billings. Its up to you to make that call

is that true for community hospitals though? Like if you join a surgical or IM group, you have to cover on-all often, and do the wards. Can you just be like sorry, only going to work 15 days a month yo. I thought posting say Fulltime or part time. So i guess 50ish hour weeks minimum but can do more if you want? Sorry, just trying to learn, most people on here are geared towards academic hospitals. Hence my questions about collingwood, St. Cats, guelph, waterloo, towns like St. Marys, that kinda thing. Thank you :).

You still have flexibility to do more or less. Perhaps not to the extremes, but the flexibility is there. Most people aren't working <40 hours because...well you're not making money.

In hospitals, you can see consults faster and get more work done. Or, you can do them slowly or cut off after a certain time, to leave the remaining work to the next guy coming on-shift.

Outside of the hospital, internists can choose to do additional outpatient clinics, or not.

Likewise for other specialties. You might be a general surgeon covering calls in your call group. But you can choose to have a hemorrhoid banding/ lumps and bumps clinic, etc.

Yeah you usually can depending on how large the hospital is. For smaller ones you may find it harder to work less as there needs to be coverage and your pool of docs is smaller. 

 

At larger community hospitals there is always a hungry young doc ready to mop up what you don’t want
