DAT Score Entry in UTDAS UofT

Hi guys,

Im a little confused on how to enter my DAT score into UTDAS. There are 3 rows, but I'm not sure what each row is for because there are no titles.

Also, in the prerequisite entry boxes, should we just put the name of the course and a link to it's course outline or should we also put the grade we received? 

Any help will be appreciated! Thanks in advance!
