Do academic physicians have any duties to publish research? Quota?

It will depend on the contract. Some merely will have teaching obligations, some will have an expected research output, but you know what the expectations are when you agree to the contract.

sure - and also academic promotion (which is different than holding clinical responsibilities) would require academic output. 

Since the department as a whole is also rewarded for academic output there can be pressure to do something as well. 

this isn't true every where but in my experience there are usual a handful of staff that are hyper active researchers, and the rest are doing "enough" not to get yelled at ha.
