Also, the way the visa system works in the states, you have to write USMLE Step3 before you can get an H1-B, and there's no way you can do it until after graduating. So that makes you rely on a J-1, which is horrible for multiple reasons. You can use an OTP year after you graduate and still remain on the F1 visa (you do not need a J1). You can write Step 3 during intern year and then apply for H1B. When you are applying for residencies you have to ask them if they even issue H1B to Canadian students who completed their training in the U.S. (they see us more favourably than IMG in some places in terms of granting visas).
