How could personality issue prevent someone from matching?

Pretty simple, being a jerk and/or not getting along with your interviewers or the team if you did an elective there.

Remember they're choosing new residents they're going to spend 2-5 years with and possibly more. If they have two similar applicants, they'll go with the one they got along with and have an easy time talking to.

Without trying to sound too corny, there's not much you can do about that just be yourself (your professional self). If you try to hard to come off a particular way it might have the opposite effect, don't be scared to show your personality.

Well, first of all, CaRMS is simply too competitive to say that someone will match to their first choice if only they have no issues. Many solid students still go unmatched, unfortunately.

That being said, as stated in your title, yes, a personality issue could hamper one's chances of matching. This could be someone who is abrasive, a poor team player, or anything that leads people in a program to not want to work with them. Sometimes such people have better chances at other schools where they are not well known.

The Program Directors are looking to hire people that they can work with for the next couple of years. A candidate might be great on paper in terms of intellectual capability, technical skills, research publications but if the program sees a conflict with personality then they won't rank that candidate highly. You'll be working with this person for years so people want to ensure they can tolerate this person for that period of time.
