Applying for jobs

Feel free to apply now, you don't have to tell them you're applying to medicine, unless you sign a contract, you can quit with notice at any time. People leave for better jobs all the time, you can work during the summer too if you want. If you are not planning to work at all if you get accepted, then maybe hold off until you find out to save yourself the trouble if you have good odds of acceptance.

You have no obligation to disclose this. Just apply for jobs and if you get in to medicine then quit. I am sure your employer would be understanding. If not, you don't owe them anything. If you have a job and a company decides they don't need you anymore, they wouldn't hesitate to let you go...
