HELP PLEASE: supplemental graduate form

I am currently a graduate student and therefore have to submit the supplemental graduate form for UCalgary's application. I was wondering if anyone has any idea what exactly is on this form? I thought it would just involve your academic advisor signing a form that indicates that you will graduate before the program starts, but my advisor randomly asked me for my resume and a CV for it? If anyone has any insight, that would be great! Thank you!!

It took my supervisor a few minutes to approve it. I sent her the link via UCANN and within 3 minutes it was "approved" by her which tells me she likely did not need a resume or CV? Maybe your supervisor assumed it was for a reference and not a grad school form verification? Sorry wish I could help more. Perhaps just let them know it is just to verify that you aren't going to just leave your current program and it is to ensure there is transparency that you are applying.
