Look at the CARMS website for timelines and eligibility. AFAIK a USMD does not need QE1 to be eligible for CARMS, just the same as Canadian MDs. You can take it at the end of 4th year if you want, after you've already matched to CARMS(if you do match). Or you can take it around the same time as STEP 2 CK, assuming you have time and energy for it around then.

This is assuming you are a true USMD at an LCME school, and not a Carrib MD.
