Fellowship in R5

With the move to Competency By Design and completing the RC exam at the end of R4, would one be able to complete a fellowship in R5? I know EM residencies have done this for a long time, but wondering if anyone has seen this in other fields (eg. Anesthesia)

At least for my field, yes that's what I heard too that depending on career objectives, 6 months to 1 year can be almost like an "in house mini fellowship". Now how recognized that "fellowship" is will vary. Like if there is a comparable fellowship that leads to Royal College/American board certification I believe most academic places will require a "real" fellowship.
