You need all those things submitted by the deadline.

 

The 48 item ABS is part of the OMSAS application. You need a verifier for each item.

 

For OMSAS, you create an account. Fill out all of the sections of the application by the deadline. Once you enter your school information, OMSAS will order a transcript from your school on their own (after you submit your completed application), you don't need to order one yourself and send it to OMSAS. You pick 3 references and have them submit the referee form and a letter of reference to OMSAS by the OMSAS deadline.

 

You don't need a CV.

 

The OMSAS application is available in July so that gives u a few months to get everything together and submit a completed application by the deadline.
