new core stream model for MD 2008

There was a discussion regarding implementation of new core stream model for PGY-1 for class of 2008 MD graduates.

 

From what I remember, residency will be integrated into 6 streams: surgery, medicine, psychiatry, pediatrics, lab medicine, and family medicine

 

Is there any new updates? How likely would be be implemented in time for 2008?

Hi there,

 

I haven't heard too many developments about this recently. The idea was bandied about last year quite a bit and I've heard folks who are for and against the notion. In short, it's an issue that still appears to be up in the air.

I heard about this a little bit. I am not completely sure, but does it pretty much involve all residents who enter the match entering one of those core streams. Then after being exposed to the different specialties and subspecialties, they compete for those positions after some training within the core stream. So, instead of applying for Urology right away, you enter the core surgery program, then after a certain number of years, you apply for Urology. Is this correct? If not, can you clarify or add anything to further explain?

I think this idea is still up in the air. We have a CaRMS infor session coming up where they're going to discuss this issue and other fun carmage stuff. I think there are a lot of issues with this approach. Like me for example who's interested in ObGyn. I assume the core stream would be surgery. But my second choice would be family, so where would that leave me if I didn't get into ObGyn after the core year?

I was under the impression that this system is unlikely to be implemented in time for the 0T9s (my class) even. I see pros and cons to the system....

Some people are likely to be unaffected (those applying for family medicine, which is a direct entry program anyway), and some are (neurology, which is a direct entry now but would I guess be under internal). From a procedural point of view, the actual content of the PGY1 year in surgery is unlikely to be affected since you rotate through a bunch of things anyway...the only downside I can see is another year where you're future is a bit 'up in the air'. One thing I was wondering about is if the complete residency program would be extended another year? Like, would we do 1 year of 'surgery', then still do 5 years once we get into whatever subspecialty + 2 years fellowship?

 

So does anyone know if programs will be extended by one year?

Hey,

 

Don't count on it happening any time soon. It'd be too expensive and too much of a bother to implement. In addition, there would be a lot of opposition to it from both the resident and consultant level.

While in theory it may be a good idea, I can't really see things changing in the near future.
