Guest tirisa Posted September 19, 2003 Report Share Posted September 19, 2003 Hiya, You'll find the forms for the referrees on the section marked "referrees" on your online application form at the OMSAS website. If you haven't yet created a login and password, then you need to do that first (and make sure you do it before October 8th). Once you're in, go to the aforementioned section, fill in the appropriate contact info, and then you'll be able to print out the confidential assessment form. Let us know if you have further difficulties. (And don't worry about being late yet... you still have time to hand them to your referrees, and have them fill it out and return it... ideally you want them to have sent it in by Oct. 15th, but your application won't be discarded if they haven't yet arrived.)
