Non Canadian and Non American-Caribbean Universities

Does anyone know: what the position is of a foreigner applying to a Caribbean med school when it comes to taking the later 2 years of in house training in the U.S.? I gathered that from the perusal info sent to me by MUA on the island of Nevis that you simply pick from the list of states given (?)

Can one just start working after completing fellowship/equivalent?

Do you become a registered doctor in the U.S., or where?

What does it mean if a Uni at the Caribbean easily accepts applications?

Can you transfer to another Uni later?
