I would include the poem (if it was published in a reasonably respectable volume) and not include the math contests. I am assuming the math is one of the Waterloo sponsored ones lots of us do in school (Pascal, Cayley or Fermat), and unless you came top three in the country or in your province, I would leave it out. Thats just my opinion however... they seem like they only want to see things pre-university that were really outstanding or extra-relevant in some way. I was planning on adding them into my diversity section hmmm... its always sort of a guess as to what to put in and what to leave out. However, if I were you, I would definitely add the poem in, and only add the math distinctions if you have alot of extra room. I think the poem shows your non-academic side (hence NAQ) better than math (which is quite academic).
