Do we just put the email that we have been communicating with them with so far? (which for me is--my utoronto email lol). Thanks for any help. It provides you with information on what exactly to write for certain boxes and identifies what needs to be filled out. I think the sample page was only provided to give you an idea of how it should be filled out but not necessarily what should be filled out nor the specifics. Someone please correct me if I'm wrong!
