I have given one of my references ample time (over 1 month) to submit a reference letter form for the UCalgary. He had previously written a reference letter for me in July this year so he has already something ready to work from. It is two days before deadline (October 15) and he has not submitted yet (I can see it in online form) - I am emailing him but not responding either.
