If you fill it out, then a third-party (like your mom, dad, friend) can correspond with the faculty on your behalf. For example, suppose you were waiting for an acceptance phone call in June and you aren't home... but your mom is, all the faculty can do is say, "I'll phone back later" unless she is the third-party you designated to have info released to -- in which case she would be told "You're in!"
