Some of the situations are not the applicant's fault, such as the referee does not use specific examples in the letter. You can compensate for that by making sure you tell them they should do so. Ideally your LOR writer knows you well enough that you can have a conversation about the letters contents in advance. If you do feel comfortable having that conversation perhaps that person is not the best person to write one for you.
