(I can see the advertisement in my head already. "Learn the 3 key knocking techniques, proper stance and body positioning, and when to stick your foot in!"

I always knock the door. Though at one particular station at UBC it didn't matter cause the interviewer already had a mess of paperwork on his table which he was going through when I entered the room, didn't even bother looking up when I entered. I had to spend forty seconds help him locate my last name in his computer.
