I know I want to pursue medicine in the future, but now I am at a crossroads with a million questions running through my head: Should I decline the PhD acceptance and take a gap year while only focusing on getting into medicine? Should I pursue the PhD program and attempt to get into Medicine while completing the PhD program? Would medicine programs frown upon my application knowing I am not done the PhD program? Should I not apply to Medicine at all until I complete the PhD?

Regardless of your stats... you should not apply to medicine while you're doing your PhD unless you have firm plans to finish your PhD. I know Calgary for example asks you to sign a waiver form saying you promise to finish your graduate program/have a plan to finish before admission to medicine. Most importantly, it's downright rude to leave a supervisor and program hanging knowing that you'd leave at the first whiff of an acceptance letter.

Seriously don't ...

I would not apply to medicine until I'm within 1 yr of finishing my PhD if I was in your position.

Couldn't agree more with the above post. But what school is this PhD at? Some schools allow you to do your grad studies and then apply to medicine leading to a combined MD/PhD. In that case, you could continue to apply for the duration of your degree instead of waiting for 3 or 4 years before applying again. It depends if you are okay waiting that long to apply again!

Don't start a PhD that you do not plan to finish - jerk move all around.  Some schools (ie UofT) would not let you apply to medicine until your final year.

Your Undergrad 3.7 GPA is not competitive for the general application pool at most schools.  Grad GPA does not come in to play at many schools.

If you are able to apply as an Indigenous applicant, you may have a strong chance at Interviews at many schools.  Your background appears to be the model candidate for NOSM   (ie. remote community, Bilingual, Indigenous, Indigenous knowledge).

If your ultimate goal is Medicine, then I would apply this cycle.    Delay the PhD.   Maybe you can still find a research related job for the gap year.

Contact a few schools directly about your application and Indigenous status.

Maybe contact this group as well for some guidance

I think if your ultimate goal is to get into medicine, I wouldn't do a PhD. You have a lot of strengths in your application that would help you get in. The other issue is time. The training in medicine is incredibly long and ultimately unless you goal is to be a clinician-scientist or something, the PhD doesn't really help you much and significantly delays you otherwise.
