Do you have a medix login? if yes: http://www.med.dal.ca for medix if you go to your "personal page" there will be a set of links on the left side and one of them should be a link that says "immunizations." or they may have a downloadable copy in your class folder. for finding the file you can use the search function located on the top left of the medix screen. i remember dal giving a paper copy of the immunization record that had to be filled out by my family doc and then later transferred to the online record in medix.
