Package to send to Ottawa by May 29th: Admission response 1000 bucks money order Copy of OMSAS response form Registration form for U of O Proof of citizenship To send to OMSAS by May 29th: OMSAS Response form Could some one please verify I've got this correctly and not missing anything?
