I'm hearing this a lot, that your elective performance, research, leadership etc that you did in med school don't really matter, and only the FM residency performance is looked at. Anyone have any info on this? The +1 program in EM is extremely competitive and I'm having a hard time imagining that your 4 years of med school are ignored entirely.

It's not ignored entirely, and someone who's gone through it can chime in, but just as your undergrad doesn't really matter anymore for residency, your performance as a FM resident is a lot more important than your previous performance as a medical student, because the expectations are different. That being said, if you did research, won awards, otherwise did something unique in medical school that would make you an asset to their program it will be looked at, but its a lot less important than the same stuff done in residency, when you have more responsibility, the work-life balance is different, and it's more relevant to actual practice. But LORs, grades, and electives from medical school are obsolete, because the FM+1 programs want to know about your performance relative to expectations of a resident, not a medical student. People can be good residents and bad med students and vice versa.
