Acceptance Package help please

Hi all, got the good news on friday, thing is I'm in poland and don't really know what's in the package and what I have to do. My parents tried to explain it to my over the phone but they tend to mess these things up.

 

Can someone please tell me the forms I have to fill out and send in by June 13.

Hi Steve McQueen,

 

Hope you're having a good time in Poland! There are three forms in the package:

 

1. 'Response to offer' form from UBC - pretty much all you have to do is confirm that you accept/decline the offer, and sign it at the bottom.

 

2. 'Application for registration with the College of Physicians and Surgeons of BC' - it asks for your contact info, a recent passport size photo, and a series of health/criminal record questions - I think you have to photocopy this, I'm a little confused, but will send a copy anyways.

 

3. 'Application for a criminal record check' - basic as well, but a $20 fee needs to be included (can be done by credit card) - a photocopy also needs to be included

 

All of these, plus a $300 deposit has to be sent to UBC meds by June 13th - maybe you can call UBC admissions and talk to them?
