Matching in the first round of CaRMS?

Does anyone know how it works when a Canadian who attends an American med school wants to apply back here for residency? Do you have to wait until the second round? Also can you apply for residency in both Canada and the States (and then decide where to go after you find out where you've been accepted), or do you have to pick between the two?

As far as applying to CaRMS from a US med school, I believe that you can only match in the second round, despite being a Canadian citizen. CaRMS is pretty explicit when it states:

 

A single match in two iterations will take place.

The first iteration will include only graduates of Canadian medical schools who have had no previous post-M.D. clinical training.
