Any vaccinations for which we have no proof of administration (even if we know we had the vaccinations) we have to have a blood test to test for the antibodies. This applies to all vaccinations, including the polio sugarcube immunization which we all may have received eons ago. As for the signatures, UofC require the doctor or nurse who has performed the immunization, or who has inspected your proof of immunization to sign the UofC form.
