Summer courses and GPA

I was wondering if anyone had any information about which med school take into account the marks for spring/summer courses when calculating GPAs and which ones don't.

<pagetext>I'm trying to find an online transcript request form, but can only find the PDF version. Is this the only version available?

 

I'm also a bit worried, as I know that even though transcripts are due on June 30, administrative errors can always happen. Realistically speaking, how long should transcripts take to arrive? If OMSAS were to receive the request form by the end of May, would I likely be in good shape transcript-wise?

 

Is there any way to track which schools have and have not received the transcripts?
