I just realized I never actually wrote my address on one the referee forms I gave to my referee.

 

will that invalidate my application?

Your name, phone number, and email address should appear automatically on each reference form that you print out, so you don't have to do anything special. As long as you've filled in your own personal details on one of the first sections of the OMSAS application, then it will automatically be there.
