What do you see as the most significant issue medicine would face in the next forty years?

I wrote how I want to practise as a family physician but how i believe the demand would go down as the need for specialists increase.

once i finished, i realized that the q'n does not ask for the most sig issue medicine would face in 'US' or 'North America', well i dont know what the trend is world wide but i still think that with advancing technology, there will be a need for specialists.

do u guyz think i'm on the right track? any suggestions, feedback?

I think you're unnecessarily limiting the scope of your response. The prompt is allowing you be broad - it's asking what's the most significant issue medicine will face. You hit on a good lead with the continued advancement of technology. Think about all the issues medicine will face in the near future: privacy issues wrt increasingly predictive medical genetic tests, our increasing ability to prolong life but not necessarily improve the quality of life, new infectious diseases, globalization, etc etc etc.

It's a wide open question!
