confidential assessment form (reference letter)

I'm staring application for the admittance in 2005 sept.

OMSAS website states that I need to have my referees send three confidential assessment forms to OMSAS but it doesn't say where I can get the form. Where can I get those?

It's great that you're thinking about your application early. Similar to you, I had started drafting my essays at this time last year. Avoiding last minute edits and frets helped.

 

The OMSAS assessment forms will be available once you register online with OMSAS, but OMSAS has not yet opened their registration process, so you won't be able to generate them yet.

 

Basically, how it works is that you enter your information as well as your three referee names. There is a section in the online application that will enable you to print the three forms, which shall be specific to each referee, i.e., each form will contain the referee's information.
