OMSAS GPA conversion chart

Hi guys,

 

I was wondering if anybody knows how OMSAS has come up with the GPA conversion chart? How much input does a specific school have with regard to which scales apply to its students in the admission process?

 

I ask because I see some inconsistencies that I would like removed.
