kaysersoze Posted June 5, 2008 Report Share Posted June 5, 2008 ok so there's 3 things: the declaration form with those 3 check boxes, the disclosure info form, and the payment form. now they're clear that the discolure and payment and a copy of ur DL must not go to teh queens office but rather to OESC..this i understand however, nothing is mentioned about the declaration form and where it should go..at least nothing that i saw... help me haha thanks in advance folks Link to comment Share on other sites More sharing options...
