Use Alberta's LOR Requirements as Example?

The University of Alberta is very specific in what they want to see in a LOR and I started to wonder if I should be using their template (4 questions - i've listed them below) for all my LOR for all universities. I figure this is one of those "it has its advantages and disadvantages" things but thought I'd ask around to see what others have done.

1.Do you feel this applicant should be admitted into the Faculty of Medicine and Dentistry? Please explain why or why not.

2.Would you have him/her as your personal physician? Please explain why or why not.

3.Please address the moral and ethical characteristics of the applicant.

4.Please comment on any/all outstanding characteristics (the strengths and weaknesses) of the applicant.
