For example, Honours B.Sc, UWO (1) Hospital Volunteer - Blah Blah (2) When I wrote my actual OMSAS application, I would write a sentance and reference it as appropriate. For example, My volunteer work at X,Y,Z (1,2,3) have given me a first-hand account of the problems patients face ..... I honestly think as long as you reference your material (when the school asks for it) you are in the clear. Just make it clear and professional and you should be fine....
