why do canadian neurosurgeons earn so much less than their american counterparts?

Just an assumption, anyone please correct me if I am wrong: Privatization and demand of the special goods/services sector. In the states, these things are less accessible (due to insurance and monetary terms, not because of the lack of neurosurgeons) to the general public and therefore the services are able to be "competitive" and pricier. Country-to-country policies also differ.
