small_town_girl Posted March 3, 2011 Report Share Posted March 3, 2011 1) Proof of Canadian citizenship or permanent residence status. 2) Two passport photos with your name nad signature on the back. 3) Photo identification (valid passport OR valid driver's license OR other acceptable identifying documents) Proof of age, if you applied as a mature student. Graduate students enrolled in a graduate program at time of application must arrange for their supervisor or the chair of their department to provide a letter indicating they are aware of this application.
