CaRMS vs. U.S National Residency Match

It is possible to submit applications to both at the same time. However, you can only be in the 'match' in one. Usually, the Canadian match happens first, and the US second. (This was not the case in 2004 when the Canadian timetable was altered due to SARS) How it works is: you submit applications to both, interview at programs in both, and then decide which match you want to enter. If you want to compete for Canadian positions, you rank your Canadian choices and the Canadian match runs. If you are matched to a Canadian spot, you are automatically withdrawn from the US match. If you are UNMATCHED in Canada, you can then also participate in the US match. However, if you matched to any program in Canada (even your last choice) you can't. If you decide that the US is really where you want to be, you withdraw from Carms and enter only the US match. You will then, hopefully, be matched in the US... however, if you aren't, the Canadian match will already be over and cannot be used as 'backup'. You will then either have to participate in the US scramble or wait a year and enter Carms again.
