Dates for Activities in Personal Attributes Section?

Should we be including dates for activities listed in the personal attributes questions. For things like employment record, I guess it would be necessary. But what about the following questions:

 

3. List and briefly describe no more than three areas where you have provided a major leadership role. (Maximum 15 lines)

 

4. List and briefly describe no more than three areas where you have demonstrated a commitment to volunteer work. (Maximum 15 lines)

 

2. List and briefly describe your most significant, top three awards or achievements. (Maximum 15 lines)

Dates might make the entries more credible and resume-like, but I don't think it would be necessary if it wasn't explicitly asked for. In the case of #4, dates might help demonstrate the commitment to volunteer work that you had.
