I believe you are just asked to read and sign the form and include it with the secondary. So yes, do include the form with the secondary. Just check your email, it should be there. For Boston U, one of the two optional questions I felt was very well addressed in my application as a whole, and it didn't make sense to me to fill it out because I thought my application really explained it all. This question was relatively specific though. Overall, most of the "do you have anything to say" questions are not specific and I almost always answer these questions. I'm confused. I can help you out b/c I learned from this mistake: sent in a Canadian check from a US dollar account. No good for RFU, they specifically asked for a money order or a US check.
