UBC MD Application - 'Institutions Attended'

I'm applying to UBC med, and I'm not sure how to enter info into the 'Institutions Attended' area of the 'Transcripts' section.

 

I took one DE&T course (Path 467) as an unclassified student (I graduated last yr from UBC) so I need to enter this info into a new line. I don't know what I should select for the 'Degree Type' or 'Program of Study'.

 

Help!!!
