Say one completed med school in Ontario and wanted to do his/her residency in B.C., what would one have to do to see that through?

Not overtly - nothing like getting into medical school in the first place. I suppose it's within the realm of possibility that programs (especially more competitive ones) might look favourably on people who are a "known quantity" to them, but that's why there are away electives.
