Ophthalmologist report for surgical specialties?

Hi guys, it seems that some schools require an "ophthalmologist report" for applying to various surgical specialties. I was wondering what the report entails and how I go about acquiring it. Do I need to see my family doc to get a referral just for this purpose? Or should I contact an ophthalmologist office directly? Thanks.
