The Medical Student Performance Record (MSPR)

I am not an expert in that field but some schools post templates of their MSPR template.
Regarding extra-curriculars, I highly doubt any schools lists them on the MSPR.
The CV and Personal statement will allow you to highlight these aspects.

No. Its a summary of all comments and evaluations from your clerkship rotations. No comparison to peers or rank.

They don't know, and these are not included. Up to you to include these on your CV or incorporate them into your personal letters.
