liszt
Anyone in primary care? They would decide who needs to go further, and who does not, I guess...

ffp
I would say family docs, and to a certain extent ER docs. In some remote areas they have nursing stations, and those nurses would be the gatekeepers. Also, some home-care nurses would be gatekeepers, because if they identify patients who aren't doing so well in the community, they might contact the patient's doctor or suggest that the patient go to emergency. Satsuma
I recently read an article discussing the identity of family medicine as a specialty and the future of family medicine as a specialty (apparently family medicine is struggling - and has been for some time - to define itself). I do not recall the name of the article, though if you did a search I am sure you would find lots of stuff. One of the roles this paper discussed in terms of family medicine was the role of family docs as "gatekeepers". But not everyone is happy with this view.
