If it's because you wanted to pursue a year of research or something like that, it may even be viewed favourably. Something out of your control, like a sick family member is probably not going to be looked at negatively. However, if you are taking a year off for personal issues like trying to regroup from a bad academic performance, I would imagine that those types of sabbaticals wouldn't be a good thing. Probably the best thing to do would be to discuss this with the academic faculty member in your med school who is responsible for your Dean's Letter.
