Graduate package for U of T and Queens

Hi guys. I heard that graduate students need to submit a separate package to University of Toronto and Queen's university. Does anyone know where I can find these packages? Is there any guide about what to include in the package?

There is nothing for Queens (Queens will do a full file review if you hit the MCAT cuts and your gpa isn't horrendous - from what i know, this means stay above 3.0)
