I think I looked like an axe murder with 90% of my hair messy and 10% lying dead across my eyes. It was a terrible photo. They still accepted it. We cannot confirm whether the photograph on the Certified Identity Confirmation Form will be accepted until we are able to review the hard copy of the document. The picture included on the form must be as indicated below. You may choose to produce these pictures yourself but as mentioned we cannot confirm whether they will be deemed acceptable until we have the hard copy of the Certified Identity Confirmation Form.
