I think the major issues are 1. once you are out in the real world making money its hard to become a resident again 2. your source of references dwindles because you no longer have supervisors seeing how great you are... But yes, most people find it best to do it right after R2 primarily for financial reasons. I know I personally would find it very difficult to be motivated to scut with a smile on my face for someone else for $60K/yr after having been staff and paid accordingly.... At least in Ontario, the MOHLTC runs it. Something like 20 family med (including pgy3s) and 20 specialty and 10 subspecialty positions are available.
