Are residents paid with T4 or can they be paid as a professional corporation?

Do residents have a choice on how they are paid? 

The standard is as an employee. Do you get a T4 tax slip or different slip?

You get a T4. As a resident you can apply to incorporate but it doesn't really make financial sense during residency.
