Unless things have changed, it is strictly P/F and many things aren't even graded. The pre-clerkship transcript literally doesn't matter, it will only show you passed every semester. In clerkship you will get a few written comments in a box on your transcript for each rotation you were on, I would say the vast majority of residency programs do not bother looking at it, but even then if you disagree with comments you can appeal to have them corrected before CaRMS.
